His usual residence was 7220 E. 17th Street, Kansas City, Jackson County, Missouri; he had lived there 2 months. He was widowed at the time of death; his spouse had been "Augusta Bierbaum." His usual occupation was "Retired Farmer; past 6 years." Cause of death was "Erysipelas Facial" (12 days duration). Contributory cause of death was "General Senility." Disease was contracted at PLACE. There was no operation prior to death. There was no autopsy. Burial, cremation, or removal was to be on 14 May 1930 at Carrollton, Missouri. The funeral director was [Rosi?] & Henderson, of 15 Jacke. Personal information was provided by Chas. Bierbaum, of Carrollton, Missouri. H. E. Songer, M.D., of 4525 Prospect, had attended the deceased from 30 April 1930 to 12 May 1930 and signed the death certificate on 13 May 1930. The certificate was received by the local registrar on 12 [sic] May 1930.
           In a letter of 7 July 1930 to the Kansas City, Missouri, Department of Health, Dr. H. E. Songer (4525 Prospect Avenue, Kansas City, Missouri) wrote:
           "In reply to your indefinite question, will say, this case of erysipelas apparently started in a pimple on the old man's face, which he was supposed to have scratched the top off of with his finger nails, and it spread from there over his face and head and down on his chest.
           "The diagnosis was made by clinical symptoms. There was no miscropic [sic] examination made. Hoping that this will be satisfactory, I remain,
           "Fraternally yours, H. E. Songer, M.D."
