We hope these tips might be of some help to you as you fill out the form.

  • Most Important: Do not use the Enter key until you have completed the form or unless you are in a multiline field, like the two line street address box or the message box.  Using the Enter key in a single line field  will submit the form before you have a chance to finish.
  • The Tab key will guide you through the form in an orderly manner.  Shift+Tab key takes you backwards.
  • You can also use the mouse to select a field box.
  • You can use the mouse to select text in a field box. After selecting the text you can use the right mouse button to delete, copy or paste the text to another field box. This may save you some typing if you plan ahead. For example, we assume that Portland will show up in quite a few city field boxes. Why not type it to the first field box, then copy and paste it to all the other field boxes that require Portland?
  • The first click in a check box inserts a check mark. Another click will remove the check mark.  Space bar does the same.
  • Most of the field boxes will hold twice the text that shows. If you type past the end of the box, the text should scroll with your typing. Use arrow buttons to scroll.
  • You can help us speed the processing of the forms. Please leave blank and delete all text in field boxes that you are not submitting. This includes the help text already in the box.  Information from empty boxes is not sent to us. This way we won’t have to wade through a bunch of n/a’s or decide if your teacher’s name is really “Fourth teacher”.
